    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • The following cold storage equipment in the Bag-Up Room were either not turned on or were unable to maintain satisfactory temperatures.- "Gluten-Free" stand up cooler- "16" stand up cooler- "MTO Sandwich" cooler- "WE-MD-09" milk dispenserEnsure that the above noted equipment can maintain a temperature of 4C or below at all times prior to storing high-risk foods inside of them. If temperature cannot be maintained, the equipment cannot be used.
    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • The dishwasher and pot washer were not turned on and ready at the time of inspection. PHI instructed that a video must be sent of both machines running with a final rinse temperature of 160F at plate level. Ensure that both machines are operating, and can reach a final rinse temperature of 160F prior to opening.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • The shelf bracing in the far corner of walk-in freezer F17 was cracked. Ensure that this shelf is removed, repaired, or replaced prior to storing food items on it. Ensure that all food equipment is maintained in good repair.
