      • PHI received a complaint regarding the cleaning and sanitizing of the freestyle drink dispensing machine.There are 2 of these freestyle drink dispensers in this facility: - one in the lobby for customer self-service that also has a built-in ice machine - one at the drive-thru window area where the ice is scooped from a bin.In the PHI's investigation, it was reported that the ice machine was not being serviced on its recommended frequency.Corrective action - arrange to have this ice machine for the drink dispenser machine in the lobby cleaned and sanitized professionally by April 17, 2026.And then to continue with the cleaning and sanitizing frequency recommended by the manufacturer and service company.

      • 1. Meat patties in a hot holding machine measured at 45 degrees Celsius. At the time of the inspection, this hot holding machine was not turned on.2. Batches of gravy in a steaming device measured at 30 degrees Celsius. The water level in the device was too low to keep food hot.These food items were discarded during the inspection. Going forward, please ensure that the temperature of hot held food items is maintained at 60 degrees Celsius or higher.

      • Hot and cold food storage temperatures were satisfactory, EXCEPT for the Front Service Counter - cream dispenser was at 10 C.Corrective Action - staff will look into this unit to see if the temperature can be adjusted, or if unit needs to be serviced, to get the temperature back down to 4 C or colder.

      • Hamburger buns were placed on the bottom shelf of the trolly. Requested to move them from the bottom shelf because its almost touching the floor surface.Bike was placed by the food trays in the storage area.Requested to move it away from food items and ensure personal items are placed away from food equipments.Fly Sticky paper strip was placed above food prep table. Requested to remove it during the inspection.

      • Water leak in the mop area from the black hose connection to the automatic mix dispenser. Requested to repair this violation within the next 4 weeks.
