      • All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT RUNNING WATER ON PREMISES; PROVIDE.
    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. MUST PROVIDE A LOG BOOK FROM A LICENSED PEST CONTROL OPERATOR WITH ALL NECESSARY DOCUMENTS: IE--MSDS SHEETS (MATERIAL SAFETY DATA SHEETS), A COPY OF THEIR STATE OF ILLINOIS LICENSE AND A MAP OF THE STORE.

      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS THROUGHOUT WHERE NEEDED.

      • The walls and ceilings shall be in good repair and easily cleaned. REPLACE ALL WATER STAINED CEILING TILES. PROVIDE A WALL COVING AT LOWER WALLBASE ON NORTH AND SOUTH WALLS. REMOVE DUST FROM VENTILATION FAN IN WASHROOM.

MUST INSTALL A UTILITY SINK WITH HOT AND COLD RUNNING WATER AND A BACKFLOW DEVICE FOR DISPOSAL OF WASTE WATER.
