- Observation:Two different cleaning cloths were found left out on the counter.Corrective Action:Wet wiping cloths must be stored in a sanitizing solution of appropriate strength between uses to prevent contamination. During the inspection, this was corrected by immersing the cloths in a quaternary ammonium compound (quats) solution at 200 ppm

- During inspection, the diamond display cooler was found to be operating at elevated temperatures of 8°C and 9°C, respectively. Temperature readings were verified using both laser and probe thermometers.Corrective Actions Taken:Kitchen staff immediately discarded potentially hazardous food items stored in the cooler, including buffalo wraps and cheesecakes.Soft drinks were left in the cooler as they are non-perishable and pose no immediate food safety risk.The site manager contacted AHS maintenance staff to address and repair the malfunctioning cooler

- During routine inspection of the cafeteria, a minor steam leak was observed in the ceiling area. There is visible condensation and staining on the ceiling insert. The site manager indicated that maintenance is aware of the issue and has placed a bucket underneath to collect the condensation. While the leak is currently minor and being monitored, it is recommended that repairs be completed to prevent the development of a nuisance condition.

- Observation:The lid for the container was found to be in poor condition, with visible cracks.Action Taken:The broken lid was discarded during the inspection to prevent the risk of plastic fragments contaminating food items and to eliminate any potential physical hazard.

Staff have been educated and reminded of the updated cleaning protocol to prevent recurrence.

Need to have a licensed pest company out to treat your kitchen for the elimination of the small flies and cockroaches.Manager updated that requisitions have gone in to seal the metal box (behind the ovens)Abell is monitoring and applying control measures (baiting, dusting, powder etc.). FM&E is in the process of tracking activity and repairing the entry points. Enviromental services is reaching out to Abell on regular basis to come and treat the area, as needed.

- Evidence of a German cockroach's infestation was found under the stainless-steel corner shelf to the right of pot sink.The PFS Supervisor noticed a bug on the floor last week, she was able to save it for identification. On July 8, a picture of the bug was sent to Abell and it was identified as a German cockroach. Abell arrived the same day and performed control services. As well, they provided a list of items to repair and clean. These are being completed today and tomorrow. Abell will be back on Friday on July 12, to monitor and apply more control measures.

Repair should be conducted immediately to prevent pest harborage and debris accumulation. 1) Ceiling in the beltline and dishwashing, area is not in good repair. Manager updated that in regard to ceiling, they are looking into options. Either adding it as a large project or putting on IMP list for repair or just replacing existing tiles with wipeable ones.2) Floor surfaces are in disrepair (rusted by the tilt skillet equipment) and taped throughout the facility (e.g. by the Suzy cue cart, dishwashing and coffee machine area). Repair or replace.3) High temperature dishwasher is leaking (minor leak). Please fix it.Appointment has been booked with a key foods company to evaluate and final repairs.The facility is currently being evaluated by the maintenance department. In the meantime, the tape of the flooring is being replaced once a week or as needed. Signs will be placed in hazardous location, preventing tripping hazards.
