- 1) Small play equipment like buttons and seashells were observed in the toddler room 6.Staff confirmed that toys are accessible to young children kids aged 3 and less.Remove all small toys from toddler rooms, that can be potential choking hazard.**Please note that a button with holes can still pose a choking hazard**2) Some of the accessible drawers in toddlers and pre-school rooms had no locks on them.Ensure that all the drawers accessible to children are always locked.3) Thumb nails were observed in toddler room 6 in drawer accessible to children.Remove thumb nails from drawer accessible to children.

- 1) Vinegar was observed in the unlocked cabinet in pre-school room 3.Violation was corrected during the inspection.2) Bingo markers were observed in the arts and craft material basket in pre-school room.Operator promptly removed the bingo markers from the facility.

- Following concerns were noted regarding the diapering protocols:1) Staff was not using the paper liners to cover the diapering surface for each client.2) Toddler room 7 had diapering mat that was visibly damaged.3) Velcro to fasten the diapering mat to its surface was either missing or ineffective in two childcare rooms on main floor and 2nd floor.**Kindly ensure that fresh paper liner is used for diaper changing.** Ensure that diapering surfaces are smooth and easily cleanable.** Ensure that diapering mats are securely attached to surface.

- Toy cleaning and sanitizing practices were not consistent throughout the facility.Some staff indicated that toys were cleaned using soap and water only, without the required sanitizing step.Ensure that the following steps are followed for toy washing and sanitizing:Wash toys with soap and water to remove visible dirt and debris.Sanitize toys by either:Soaking them in a bleach solution at 100 ppm, orSpraying them thoroughly with an approved sanitizer.(Ensure the required contact time for the sanitizer is met.)Air‑dry toys completely before reuse.For larger toys that do not fit into the toy‑washing sink, the clean‑in‑place method must be used.Alternatively, toys may be washed and sanitized in a dishwasher using a sanitizing cycle, where appropriate.
