      • 1) Small play equipment like buttons and seashells were observed in the toddler room 6.Staff confirmed that toys are accessible to young children kids aged 3 and less.Remove all small toys from toddler rooms, that can be potential choking hazard.**Please note that a button with holes can still pose a choking hazard**2) Some of the accessible drawers in toddlers and pre-school rooms had no locks on them.Ensure that all the drawers accessible to children are always locked.3) Thumb nails were observed in toddler room 6 in drawer accessible to children.Remove thumb nails from drawer accessible to children.

      • 1) Vinegar was observed in the unlocked cabinet in pre-school room 3.Violation was corrected during the inspection.2) Bingo markers were observed in the arts and craft material basket in pre-school room.Operator promptly removed the bingo markers from the facility.

      • Following concerns were noted regarding the diapering protocols:1) Staff was not using the paper liners to cover the diapering surface for each client.2) Toddler room 7 had diapering mat that was visibly damaged.3) Velcro to fasten the diapering mat to its surface was either missing or ineffective in two childcare rooms on main floor and 2nd floor.**Kindly ensure that fresh paper liner is used for diaper changing.** Ensure that diapering surfaces are smooth and easily cleanable.** Ensure that diapering mats are securely attached to surface.

      • Outstanding:The first aid kit is missing single use-antibiotic ointment. Please ensure the first aid kits are equipped with necessary supplies.May 8th, 2026: Operator has ordered the antibiotic ointment. Kindly update PHI once it is available.

      • Facility needs cleaning around the corners and storage shelves in multiple rooms, included but not limited to toddler room 7, toddler room 4 and pre-school room 3.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Toy cleaning and sanitizing practices were not consistent throughout the facility.Some staff indicated that toys were cleaned using soap and water only, without the required sanitizing step.Ensure that the following steps are followed for toy washing and sanitizing:Wash toys with soap and water to remove visible dirt and debris.Sanitize toys by either:Soaking them in a bleach solution at 100 ppm, orSpraying them thoroughly with an approved sanitizer.(Ensure the required contact time for the sanitizer is met.)Air‑dry toys completely before reuse.For larger toys that do not fit into the toy‑washing sink, the clean‑in‑place method must be used.Alternatively, toys may be washed and sanitized in a dishwasher using a sanitizing cycle, where appropriate.

      • - Toys were observed being stored in the designated diapering/handwashing sink in one of the rooms. The room has a separate dedicated sink for cleaning purposes. Ensure that items are not stored in the handwashing sink at any time.*Staff are to discard all paper and other absorbent play materials found in the sink. Non‑absorbent, smooth, and easily cleanable toys must be cleaned in a separate area using the wash, rinse, and sanitize method prior to being returned to use.

      • - The wall behind the first-floor stairs door appeared to be damaged at the time of inspection.*Ensure al the surfaces are smooth, non-absorbent and easily cleanable.**Repair the wall.

      • Staples were used in the classrooms. Staples can fall on the floor and pose a safety hazard to the children, including the risk of injury. Please use sticky tags, tape or other child-safe alternatives instead.

      • The first aid kit is missing single use-antibiotic ointment. Please ensure the first aid kits are equipped with necessary supplies.

Please ensure that children's personal items, including jackets, are separated at all times. Each child's jackets should be stored individually.

      • Cleaning rags were hung on the faucet. Please ensure the following:- cleaning rags are used only once and then disposed of or laundered. This prevents cross-contamination and bacterial spread. - place clearly labeled bins in each classroom for used rags.

      • 1) Cabinet door below the sink had fell off the hinge (Toddler 2). -repair the cabinet door.2) Walls and window sill areas were damaged. The window sill was currently being repaired with drywall mud (Toddler 2). -continue routine repairs of surfaces. If there are walls or other surfaces that are constantly damaged, considering installing FRP panels since they are much more durable than drywall, but also easy to wash/clean.

      • In Toddler room 7, the change table staps were broken.** Repair or replace so the safety belts are able to be used as intended.

      • In the preschool room 2 washroom, the wall near the paper towel dispenser was unfinished. **Apply a proper finish over the wall surface so it is made smooth, non-absorbent to moisture, and easily cleanable.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There was no paper towels in the paper towel dispenser for the sink in the Preschool room 4 (near the washroom).**Replenish hand washing supplies. **This was corrected during the inspection**

      • Some of the first aid kits were missing CPR barriers and antiseptic wipes. ** Please update all first aid kits to include the CPR barrier masks and antiseptic wipes. **This was corrected during the inspection**

      • 1. In the preschool room 4 washroom, some of the cabinet handles were loose and one was missing.**Please resecure the cabinet handles and install the missing handle.2. The inner seal of the washing machine in the laundry room had a buildup of grime/debris accumulating within the crevices.**Please clean /disinfect the indicated area.

      • Some of the bottles in the Toddler 7 room was missing names on them. **Please label the bottles with the child's name. **This was corrected during the inspection**

Bleach in sink used for toy sanitizing measured 500ppm instead of the required 1000ppm during outbreak.- Corrected.Please ensure toys are fully submerged in the bleach solution instead of sprayed to ensure adequate surface contact.

      • Some rooms only had one disinfectant spray bottle that was being used for tables/toys and diapering.- Ensure there are two designated disinfectant bottles, one for tables/toys, and a separate one for diapering.Disinfectant spray bottles in some rooms had broken nozzles.- Ensure disinfectant spray bottles are working properly.
