      • 1. A live duck was discovered during the routine inspection. AHS was not notified about the duck and written approval was not obtained.- Written approval is required for live poultry, chickens, turkeys ducks, and geese.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Staff was unable to provide proper toy washing steps when questioned. A sanitization step was missing. Vinegar is not a sanitizer.- Education was provided on site. Please review the guidelines and provide additional training to staff.
