    • 03. Is all food in this facility fit for human consumption and not unwholesome, spoiled, or adulterated?
      • -Cut bok choy leaves in the salad kept in the front prep cooler were wilted and spoiled. -Food items such as bok choy and sprouts in the facility did not seem to be fresh and edible.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Houselfies (dead and alive) were observed in the back storage area of the kitchen.There is no mention or evidence of pests in the report provided by the pest control operator.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Unsanitary way for cleaning woks in the front prep area was observed. The scrubber used for cleaning woks was unsanitary and had accumulation of food debris and grime.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • -A buildup of food debris and dirt was observed in hard-to reach areas of the back storage, walk-in cooler, and the food prep area.-The surface of the prep cooler in the front was kept unsanitary.-The shelves of the table used for keeping files had food debris all over.-The cooking equipment such as rice cooker and containers for keeping food were dirty and insanitary.-The uncooked rice was all over the floor in the back storage area.-The back storage area and the dishwashing area was disorganized. It is difficult to access and clean hard-to-reach areas due to the accumulation of clutter.
