- Corrective Action: The operator immediately stopped using the cutting boards. Repair or replace worn out equipment to ensure all food contact surfaces are smooth, non-porous, and easily cleanable.

- Observation: (CORRECTED DURING INSPECTION) Soda drinks in the slurry machine were melted and have been kept in room temperature for a long time. - Corrective Action: Operator immediately discarded the soda drinks.

- Observation: (CORRECTED DURING INSPECTION) Some bananas stored in open area are peeled off from the stem. Deep fryers were accumulated with food debris. Some utensils in a box were stored on the ground. (Public Health Significance) - Fruits that are peeled off are exposed and prone to contamination, promoting bacterial growth in room temperature. Food debris accumulated on deep fryer can promote bacterial growth, potentially increasing the risk of foodborne illnesses. Storing utensils on the ground will increase the risk of food contact surface contamination.
- Corrective Action: The operator immediately discarded the bananas that have been peeled off. The operator immediately cleaned the deep fryer and filtered the oil. The operator immediately lifted the utensils off the ground.

- Observation: (CORRECTED DURING INSPECTION) The dishwasher's chlorine sanitizer did not reach to the required concentration of 50ppm. (Public Health Significance) - inadequate sanitizer concentration results in ineffective food contact surface sanitation, potentially causing foodborne illnesses.
- Corrective Action: Operator immediately fixed the dishwasher and the concentration of the sanitizer reached to 100ppm.

- Observation: (CORRECTED DURING INSPECTION) A bag of potatoes have budded. (Public Health significance) - budded potatoes contain glycoalkaloids which act as neurotoxins. The toxins are heat stable and can cause food intoxication.
- Corrective Action: Operator immediately discarded the potatoes. Operator to ensure potatotes are freed from budding.

- Corrective Action: Operator immediately discarded the stran skimmer. Operator to ensure all utensils are in good working condition.

- Corrective Action: The operator immediately discarded the desserts.

- Observation: (CORRECTED DURING INSPECTION) A can of tomato was opened and stored near the sink. (Public Health significance) - putting food too close to the sink might increase the risk of food contamination by water splashes.
- Corrective Action: Operator immediately moved food away from the sink.

- Observation: (CORRECTED DURING INSPECTION) Breads in the walk-in cooler were not properly covered. Meats were found stored on the floor of the walk-in freezer. Ice accumulated on the floor and ceiling of the walk-in freezer. Public Health significance) - dust and ice might harbor pathogens that could subsequently contaminate uncovered foods. Foods stored on the floor might subject to pest and waste water contamination.
- Corrective Action: Operator immediately covered the breads and removed the meats from the floor. Operator immediately removed the ice.

- Observation: Light tubes in the kitchen were not covered. (Public Health significance) - lights should be covered to prevent physical contamination to food when shattered.
- Corrective Action: Operator to re-install all light covers.

- Observation: (CORRECTED DURING INSPECTION) A knife was found in the handwashing sink. A white cutting board was discolored. Utensils such as spatulas and laddles were found damaged. Plastic trays for the dishwasher were accumulated with discolored scale deposits. (Public Health significance) - Food contact surfaces and trays contacting clean utensils should be kept in good working conition, such as smooth and non-absorbent, for effective cleaning and sanitizing. Soiled utensils stored in handwashing sink can be heavily contaminated that makes subsequent cleaning and sanitizing ineffective.
- Corrective Action: Operator removed the knife from the handwashing sink. Operator removed the white cutting board for resurface. Operator discarded damaged utensils. Operator must switch to manual dishwashing before the dishwashing trays are cleaned or replaced. Operator replaced the dishwasher trays the same day.

- Observation: Food debris was found on the meat slicer and the interior of the microwave oven. Dust accumulated on the fan covers in the walk-in cooler. Burnt residues adhered to all over the internal surfaces of ovens. Plastic trays for the dishwasher were accumulated with discolored scale deposits. (Public Health significance) - food debris and burnt residues can harbor pathogens that can contaminate foods. Burnt residues and scale deposits could prevent effective cleaning and sanitization.
- Corrective Action: Operator to clean, scrub and disinfect the meat slicer and ovens thoroughly and frequently. Operator to clean the fan covers and ensure it is free from dust accumulation at all times.

- Observation: Some walls, back doors, floor and shelves were peeled off. (Public Health significance) - all surfaces in the food premises should be clean, smooth and non-absorbent to prevent dirt accumulation and allow effective cleaning.
- Corrective Action: Operator to ensure all surfaces in the premises are smooth and non-abosrbent.
