      • One roll of stickers for cups had mouse droppings in the bag.-Instructed operator to discard the stickers
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Wooden plank used as a counter beneath the pearls was bare (absorbent) and had multiple cracks in it (not smooth). Pearls and syrup were stuck to it.-Remove wood plank. Use a material that is smooth and non-absorbent

      • **Continuing violation**Edges of the floor and hard-to-reach areas beneath cabinets have an accumulation of debris.-Thoroughly clean the facility

      • Sanitizer spray bottle not labeled.-Ensure all containers are labeled
    • 15. Is the facility free of a pest infestation?
      • Green mouse droppings found throughout the cabinet beneath the bubble tea preparation counter where syrups and other products are stored. Edges of the floors also had droppings. Operator reported not having regular pest monitoring.-Contract a pest control company to eliminate the infestation-Safely clean mouse droppings (instructions in opening comments, also provided onsite)
