      • A bunch of plastic fibers were noted in the room. Optic fiber needs to be removed due to entanglement hazard.

      • Childrens hand sinks in the rooms were not consistently producing warm water for handwashing. Ensure the hand sinks are producing warm water for handwashing.

      • **100ppm chlorine sanitizer was noted being used for disinfecting the diapering surfaces in the facility. Facility was aware of 1000ppm chlorine sanitizer requirements. 1000ppm bleach sanitizer was prepared during inspection onsite. All sanitizer bottles were labeled “1000ppm sanitizer solution”

      • Hand washing sink located in the shared bathroom between room 1 and 2, was not supplied with paper towels. Ensure all handwashing stations are supplied with hot and cold running water, soap and paper towels.

      • There was a build up of debris underneath the diapering station mat located in the washroom between the senior toddler and preschool rooms. Ensure that the area underneath the mat is cleaned on a regular basis.

      • The test papers used to measure the chlorine sanitizer concentration expired on April 2023. The concentration of different sanitizer containers around the facility were adequate when measured with the PHI's own test strips. Ensure that new test strips are acquired so that staff can reliably measure the concentration of sanitizer solutions.
