EMPLOYEE CALLED MR HAMODEH WHO STATED THAT HE CALLED SEVERAL TIMES ABOUT THE CERTIFICATE AND THEY HAVE NOT MAILED IT TO HIM. INSTRUCTED HIM TO FOLLOW UP WITH THE INSTRUCTOR. SERIOUS VIOLATION 7-38-012 ISSUED.
