      • Observed front bar exposed hand sinks blocked and inaccessible at time of inspection with dump sink strainer. Also observed no paper towels at said exposed hand sink. Instructed manager that exposed hand sinks must be accessible and with paper towels to ensure proper hand-washing. Manager corrected violation during inspection. Critical violation #7-38-030

      • Observed slight calcium stain deposits on interior ice curtain on both ice makers in basement dish washing area. Instructed manager to detail clean and maintain at all times.

      • Observed broken glass and debris on floor in beverage closet in basement storage area. Instructed manager to detail clean and maintain at all times. Must install splash guard on left side of exposed hand sink, at lounge bar area to prevent contamination. Maintain at all times.

      • Observed beverage inventory stored on floor in basement elevator closet. Instructed manager to elevate items at least 6 inches off the floor at all times. Maintain said.

      • Observed 2 floor hand sinks with strainer and limes inside basin making inaccessible. Observed no soap or paper towels at both bar hand sinks on 2nd floor. Observed no soap or paper towels at 1st floor bar hand sink; Observed drain lines leading into hand sink from bar drink rail (improper use of hand sink). Observed no soap or paper towel at bar hand sink. Observed no soap at hand sink in dish area; Observed no soap at hand sinks in basement prep area. Instructed all hand sinks must be stocked with soap and paper towels (or hand dryers) at all times. Instructed hand sink may only be used to wash hands. Instructed hand sinks must be easily accessible at all times. Operator provided soap and paper towels for all hand sinks during inspection. Operator removed items stored inside hand sinks. Critical violation 7-38-030.

      • Observed potentially hazardous foods not properly thawed. Found raw, frozen goat stored in sink basin in standing water. Instructed must follow an approved method of thawing such as storage in a refrigerator @ 40F or less; under portable running water at a temperature of 70F or below; or in a microwave oven (only when the food will be immediately transferred to a conventional cooking facility to continue the cooking process). Serious violation 7-38-005(a).

Instructed must post original city of Chicago food manager certificate in plain view of customers at all times. Instructed must have city of Chicago certified food manager on site at all times when potentially hazardous foods are being prepped, cooked and/or served. Serious violation 7-38-012.

      • Observed slow draining hand sink in north bar on 2nd floor. Must repair and maintain. Must install at minimum 1 compartment sink in both bars on 2nd floor to use for rinsing and dumping/discarding beverages. Must maintain all plumbing. Must install splash guard on the left side of exposed hand sink in dish area to prevent contamination (clean dishes stored on a shelf to the left of the sink).

      • OBSERVED THAT THE INTERIOR BAR RAILS AT ALL THREE BARS (1ST FLOOR, 2ND FLOOR FRONT AND 2ND FLOOR REAR) ARE UNFINISHED AND EXPOSING UNSEALED WOOD FRAMING. INSTRUCTED TO PROVIDE A SMOOTH, NON-ABSORBENT AND EASILY CLEANABLE SURFACE AT THE INTERIOR BAR RAIL.

      • OBSERVED PORTIONS OF CEILING THAT MUST BE PATCHED AND REFINISHED IN THE FOLLOWING AREAS: THE CEILING ABOVE THE 1ST FLOOR BAR, THE BASEMENT WOMEN'S RESTROOM, THE HALLWAY OUTSIDE THE WOMEN'S RESTROOM. INSTRUCTED TO REPAIR THESE AREAS. MUST FINISH THE WALL NEAR THE FRONT MAIN BAR WHERE THE LIGHT SWITCHES TO BE NON-ABSORBENT AND EASILY CLEANABLE. ALSO, MUST PROVIDE LIGHT SWITCH COVERS, RESTROOM SPEAKER COVERS, AIR DUCT COVERS, AND ACCESS BOX COVERS WHERE MISSING.
