      • Bleach solution used within the facility tested at approximately 500 ppm, exceeding the required concentration of 100 ppm. Bleach solutions that exceed the required concentration may pose a chemical exposure risk.Actions Taken:- A new bleach solution at 100 ppm was prepared by staff during the inspection.- The operator was educated on preparing bleach solutions at the correct concentration and verifying concentrations using appropriate test strips prior to use.

      • Debris was observed underneath a mat in the infant room. Mats and surrounding areas must be maintained in a clean and sanitary condition to prevent contamination and reduce the risk of illness.Action Required:- Ensure mats, including the areas underneath and around them, are cleaned and disinfected regularly.

      • A sofa in the out-of-school care room had torn upholstery. Furniture must be maintained in good repair, as damaged furniture cannot be effectively cleanedAction Taken:- The damaged sofa was removed from the room by the staff during the inspection.

      • 1. Children's tables and chairs were observed with chipped paint. Furniture must be maintained in good repair and in a condition that allows for effective cleaning and safe use by children.2. The wooden water bottle storage rack in the toddler room was water damaged and had peeling surfaces. Equipment must be maintained in good repair and be easily cleanable.3. Peeling paint was observed along the lower edges of walls in the common area where baseboards were missing. The operator stated they are aware of the issue and are in the process of arranging repairs.Action Required:- Repair, refinish, or replace the affected tables, chairs, storage rack, walls, and baseboards to ensure surfaces are smooth, easily cleanable, and maintained in good repair.

      • 1. Staff personal belongings were stored in cabinets accessible to children in Preschool Group 1 room. There may be potential hazardous items including personal medication inside the purses.The personal belongings were removed from the cabinets immediately and moved to staff lockers.

      • 1. The surface for one of the toy cabinets in Preschool Group 2 room was in disrepair and not easily cleanable. The operator will be replacing it with a new cabinet.
