      • ***Not reviewed - Pedicure station was not prepared for review***Staff stated that when disinfecting pedicure basins, Preempt RTU is diluted into water as a step in the disinfecting process.Disinfecting Preempt RTU is contradictory to the manufacturer's instructions for use and dilutes the disinfectant, rendering it ineffective.

      • - The sink in the media room was not draining well.- Numerous sinks were missing either paper towel dispensers and/or soap dispensers.- Hot water pressure was very weak.

      • ***Update - February 27, 2026*** The operator provided two separate certificates or the same laser. One certificate indicated a class 4 laser and the other indicated a class 3 laser. Contact AASP to arrange for another inspection and confirm if the laser is a class 3 or 4. Certificate for the class 4 laser was not reviewed.Provide the certificate for the class 4 laser for review.

      • Written procedures were not available.Ensure that required written procedures are readily available for staff reference, and for Public Health Inspector review.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Several bottles/containers of 70% alcohol were available in the facility. None contained a DIN or NPN. Skin antiseptic must have a DIN or NPN and must be indicated for use as a skin antiseptic.

      • Staff stated that Preempt HLD 5 and/or Preempt CS20 is used up to the manufacturer's maximum stated days of use without any testing to confirm if solution is still acceptable for use after the first day.HLD 5 and/or CS20 may be reused beyond one day of use if solution is tested, is of an acceptable concentration, and logs tracking reuse and testing are maintained.

      • Staff stated that HLD 5 and/or CS20 and/or alcohol were being used for surface disinfection.- HLD 5 and CS20 are indicated for disinfecting reusable semi-critical tools, not for surface disinfection.- Alcohol is not indicated for use as a surface disinfectant.
