- ***Update - February 27, 2026*** The operator provided two separate certificates or the same laser. One certificate indicated a class 4 laser and the other indicated a class 3 laser. Contact AASP to arrange for another inspection and confirm if the laser is a class 3 or 4. Certificate for the class 4 laser was not reviewed.Provide the certificate for the class 4 laser for review.

- Written procedures were not available.Ensure that required written procedures are readily available for staff reference, and for Public Health Inspector review.

- Several bottles/containers of 70% alcohol were available in the facility. None contained a DIN or NPN. Skin antiseptic must have a DIN or NPN and must be indicated for use as a skin antiseptic.
