  1. Risk Management Inspection

    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • The operators are providing laser treatments, light treatments, cosmetic puncturing. No personal service notification was submitted. Submit a complete personal service notification to ensure all services are reported.
    • 20. Is reusable equipment disinfected properly?
      • The operators are using alcohol to disinfect the attachment pieces for the microdermabrasion. Non-critical items such as the different attachment pieces for the microdermabrasion equipment must be disinfected with at least a low-level disinfectant with a DIN. Please use a low or intermediate-level disinfectant and refrain from using alcohol as it is not considered a disinfectant.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Several inks were expired and stored in the drawers. Tattoo inks should not be used when expired as the product cannot be guaranteed safe to use past the expiration date. Ensure tattoo inks are regularly checked for expiration dates and discard those that are expired.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • The operators are using NumbSkin Lidocaine 10.56% and it does not have a DIN/NPN. Lidocaine must have an active DIN/NPN. Lidocaine without a DIN/NPN is prohibited to use. Obtain a lidocaine with a DIN/NPN.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures are not available for decontamination of a dual purpose sink. Where a sink is installed and used for both hand washing and reprocessing equipment a written procedure for sink decontamination in between use must be completed and adhered to. An AHS template was provided to the operator.
