      • ** Repeat Violation**- April 30, 2026June 20, 2024:Multiple jars of barbicide had lots of hair debris at bottom.-Ensure staff are following manufacturer's instructions for use of barbicide (cleaning tools before disinfection)
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Surfaces including chairs and hair washing sink of some stations were observed with hair and debris.- Thoroughly clean and maintain in a sanitary condition at all times. Ensure that surfaces are disinfected with at least a low-level disinfectant.
