    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The shared reprocessing area was observed to be located in a bathroom with a toilet.Facility must have an appropriate reprocessing area that does not contain a toilet and not be in an area that incompatible activities/undertakings are occurring. **Note: Operator indicated that they may be able to utilize the two-compartment sink in the basement as the dedicated reprocessing area for their semi-critical equipment (metal tweezers for eyelash application).**Update (January 7, 2026): Operator indicated that they have moved their equipment reprocessing area into the basement where the two-compartment sink is located. A re-inspection must be conducted by Public Health Inspector for review/verification.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures were available on site for cleansing skin, reprocessing sink decontamination, applying antiseptic and accidental skin cuts and punctures. AHS written procedure templates were provided to both PSE operators during the inspection.Ensure that the written procedure templates is completed and kept on site at all times.Operator (Dream Beautified Esthetics) completed required written procedures templates and submitted them to PHI via email the same day of the inspection, however only written procedures for Cleansing Skin was completed. Please complete remaining written procedures documents (Accidental Skin Cuts and Punctures and Reprocessing Sink Decontamination)Operator (Iconic Beauty by Rose) is still required to complete written procedures (Cleansing Skin, Reprocessing Sink Decontamination, Applying Antiseptic and Accidental Skin Cuts and Punctures) templates and submit them to PHI for review/verification.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The following anesthetic was observed to be expired and had no DIN or NPN in the Iconic Beauty by Rose service area: 1. Blue Gel Anesthetic 1 oz. EXP: 04/2025The following anesthetic was observed to be expired:2. Zensa Lidocane Cream 30 g (DIN 02449684; EXP 03/24)Operator discarded products into the garbage during the inspection. All antiseptic products used on the skin must contain a DIN/NPN verifying that the product is approved for use by Health Canada and must be unexpired.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • The following single use tattoo needle cartridge (2 pack) was observed to be expired in the Iconic Beauty by Rose service area: 1. HYVE Beauty - Dream Luxx Tattoo Needles ; LOT E0020945 - EXP 23/10/2025Operator discarded into appropriate sharps container during the inspection.Ensure that equipment that is meant to be sterile at the point of use for invasive/high risk procedures are unexpired and used in accordance with the manufacturers instructions for use.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 10 open bottles of brow tattoo pigment ink (Tina Davis - Perma Blend) in various shades were observed to be expired in the Iconic Beauty by Rose service area. Expiration dates observed on bottles: 04/06/2025 and 06/04/2025. Operator discarded products into the garbage during the inspection. Ensure that cosmetic products used for invasive/high risk procedures are unexpired and used in accordance with the manufacturers instructions for use. **Note: Manufacturers website states that ink pigments have a shelf life of 1 year after being opened and 3.5 years unopened.
