      • Monitoring for pest is taking place but not being recoded. Records had been completed in the past and staff located and completed the necessary checklist.

      • Continues: There is a gap at the base of the front door to the facility. This gap could allow pests to enter.As of January 29, 2025, the facility operator indicated that this has been an ongoing problem. The building shifts throughout the year, and the formation of the gap is due to foundational issues (a building inspection has been conducted).The back door leading directly into the kitchen was free of any visible gaps/entry points for pests. Checklists are completed each month.Please continue to monitor the gap and take action to block it as necessary.
