      • 1. Several walls throughout the facility and classrooms are chipped and in disrepair due to repeated impact from cots and other equipment. The damaged wall surfaces are not maintained in good condition.The operator is aware of the issue and stated that the walls will be refinished.September 22, 2026: The facility is in the process of refinishing the walls.

Build-up of debris under the diapering mats located in the following rooms:- The diapering mat located in the bathroom for Grasshopper Room- Underneath the diapering mat located in the butterfly room.Please ensure staff clean and sanitize under the diapering mats.2. The garbage bin located next to the handwashing sink in the Caterpillar Room is in disrepair. The pedal-operated lid does not function properly and does not consistently open when pressed.Repair or replace the garbage bin to ensure it is maintained in good working order and can be operated hands-free to support proper hygiene practices.

      • The hand sink in the chipmunk's room was measured at a 18.8C. Please ensure hand sinks are capable of providing warm running water to promote handwashing. AHS guidelines recommend a temperature of 49C and cannot exceed a temperature of 54C.Sept 22, 2026: Please set at a comfortable temperature around 38 degrees C.

      • 1. Toys that were not appropriate for the age group were accessible in the Busy Bees Room. Dot markers labelled for children ages 3+ were stored in the classroom, and children were observed playing with small, thin-shaped toys that may pose a choking hazard. Staff confirmed the toys were intended for children aged 3+ and had remained in the room from a previous age-appropriate cohort. Staff removed the toys during the inspection.Ensure all toys and materials accessible to children are developmentally appropriate for the age group in care and do not present a choking hazard.2. cleaning supplies and chemicals in the starfish room were stored in a drawer without a proper lock.Please install a lock to ensure all cleaning supplies are inaccessible to kids to eliminate chemical hazards

The garbage bin located next to the handwashing sink in the Caterpillar Room is in disrepair. Chlorine sanitizer concentrations were not consistently maintained throughout the facility. - The bleach for the Indoor Climber Gym and Sports Gym were below the required levels. - In the Grasshopper Room, the bleach sanitizer measured 50 ppm- The sanitizer bottle labeled as 100 ppm in the Squirrel Room measured 1000 ppm.- the Bleach in the sea horse room was measured at 1000 ppmEnsure all chlorine sanitizer solutions are prepared and maintained at the appropriate concentrations. Verify sanitizer concentrations using test strips and remake solutions as needed. Staff must be trained on proper sanitizer mixing procedures to ensure effective cleaning and disinfection.

      • 1. Expired 1000 ppm chlorine test strips were noted in the Discovery Zone Room. The Discovery Zone Room is located in close proximity to the Indoor Climber Gym and Sports Gym, where sanitizer concentrations were found to be below the required levels during the inspection.Ensure test strips are within their expiry date and are used to routinely verify sanitizer concentrations throughout the facility.

      • 1) Soap in the bottom drawers by the two-compartment sink located in the Starfish Room, and Sunbeams Room were not equipped with locks and were accessible to children.The operator promptly relocated the soap into a locked cupboard.2) The safety locks on the chemical cupboard beneath the two-compartment sink in the Foxes Room were not fastened.The operator promptly locked the cupboard.3) Safety locks were missing from the chemical cupboard under the two-compartment sink located in the Starfish Room.The operator promptly installed new safety locks.

      • The wire rack attached to the wall of the playhouse area located in the Discovery Zone was observed to be broken exposing sharp wires.Please completely remove the wire rack or replace it to prevent accidental punctures.

      • 1) The faucet valves in the play kitchen located in the Discovery Zone were observed to be cracked exposing sharp edges.The operator promptly removed the entire faucet from the play kitchen to prevent accidental injuries.2) Broken shells were observed in the Foxes Room.The operator promptly discarded the broken shells. Please monitor the shells to ensure that broken shells are removed immediately to prevent cuts and punctures.3) The lids of sensory bottles filled with tissue paper and cut up straws located in the Sunbeams Room were not securely fastened.The operator promptly removed the bottles from the room. Please glue the lids onto the sensory bottles to prevent potential choking hazards.

      • 1) The diaper strap located on the diapering station in the Sunbeams Room was observed to be soiled in white crust.The operator promptly cleaned and disinfected the strap.2) Dirt and sand were observed underneath the changing mat of the diapering station located in the Busy Bees Room.The operator promptly cleaned and disinfected the surfaces underneath the changing mat.
