      • 1. The cleaning schedule was last completed December 5th. The staff member stated cleaning and sanitizing is being completed daily. It was difficult to verify completion.Complete the cleaning schedule for the week of December 15 -December 19. Please email a copy of the completed list on December 22nd.Please ensure the checklists are completed daily.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1) Soap in the bottom drawers by the two-compartment sink located in the Starfish Room, and Sunbeams Room were not equipped with locks and were accessible to children.The operator promptly relocated the soap into a locked cupboard.2) The safety locks on the chemical cupboard beneath the two-compartment sink in the Foxes Room were not fastened.The operator promptly locked the cupboard.3) Safety locks were missing from the chemical cupboard under the two-compartment sink located in the Starfish Room.The operator promptly installed new safety locks.

      • The wire rack attached to the wall of the playhouse area located in the Discovery Zone was observed to be broken exposing sharp wires.Please completely remove the wire rack or replace it to prevent accidental punctures.

      • 1) The faucet valves in the play kitchen located in the Discovery Zone were observed to be cracked exposing sharp edges.The operator promptly removed the entire faucet from the play kitchen to prevent accidental injuries.2) Broken shells were observed in the Foxes Room.The operator promptly discarded the broken shells. Please monitor the shells to ensure that broken shells are removed immediately to prevent cuts and punctures.3) The lids of sensory bottles filled with tissue paper and cut up straws located in the Sunbeams Room were not securely fastened.The operator promptly removed the bottles from the room. Please glue the lids onto the sensory bottles to prevent potential choking hazards.

      • 1) Single-use paper towels were unavailable at the washroom handwash stations located in the Starfish Room, the Discovery Zone.The operator promptly refilled the dispenser with single-use paper towels.2) Hand soap was unavailable at the washroom handwash station located in the starfish room.The operator promptly refilled the dispenser with liquid hand soap.

      • The mini cooler temperature located in the Busy Bees Room was tested using an infrared and probe thermometer and measured at 6.8C.The operator adjusted the temperature, and the cooler was rested measuring at 4C.

      • 1) The diaper strap located on the diapering station in the Sunbeams Room was observed to be soiled in white crust.The operator promptly cleaned and disinfected the strap.2) Dirt and sand were observed underneath the changing mat of the diapering station located in the Busy Bees Room.The operator promptly cleaned and disinfected the surfaces underneath the changing mat.
