    • 20. Is reusable equipment disinfected properly?
      • 1) Several containers of Cavicide (Hydrogen Peroxide) throughout the facility had expired.REQUIREMENT: Do not use/discard the expired product.2) One laser hair removal device handpiece comes into direct contact with skin but is not being disinfected (currently only 70% alcohol is being used).REQUIREMENT: Contact the manufacturer and verify which disinfectants could be used on the machine. Report back to the Inspector with the results and next steps.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Two syringes located inside a refrigerator storage box (same box where the hyaluronidase was kept) had expired.These syringes were promptly discarded.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • A vial of hyaluronidase had been used, but the date was not added to the vial. After opening, hyaluronidase had 30-day shelf life according to the instructions for use.A record was able to be found for when the vial was used, and a date was added to the vial. Product was not expired.

    • 20. Is reusable equipment disinfected properly?
      • The facility has been providing extractions during facials using comodone extractors, a semi-critical tool. There was no high-level disinfectant available to disinfect the tool after use; the facility was using an intermediate-level disinfectant (Cavicide).This matter is considered corrected during the inspection since the facility will switch to extractions using a hand.REQUIREMENT: If the extractor tool is to be used, then a high-level disinfectant must be obtained.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Botox is not being reconstituted with a non-preservative free saline solution, which is contrary to the manufacturer's instructions for use.REQUIREMENT: Obtain preservative-free saline for reconstitution.
