    • 20. Is reusable equipment disinfected properly?
      • The facility has been providing extractions during facials using comodone extractors, a semi-critical tool. There was no high-level disinfectant available to disinfect the tool after use; the facility was using an intermediate-level disinfectant (Cavicide).This matter is considered corrected during the inspection since the facility will switch to extractions using a hand.REQUIREMENT: If the extractor tool is to be used, then a high-level disinfectant must be obtained.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Botox is not being reconstituted with a non-preservative free saline solution, which is contrary to the manufacturer's instructions for use.REQUIREMENT: Obtain preservative-free saline for reconstitution.
