      • Single use equipment such as emory boards were being reprocessed and were observed in the reprocessing area.**Ensure all single use equipment are discarded immediately after use.
    • 20. Is reusable equipment disinfected properly?
      • Some disinfectants are not being used according to the manufacturer's instructions. Staff indicated that the mixed barbicide solution is used for up to 14 days and the Preempt RTU disinfectant was observed to be expired.**Ensure a fresh solution of barbicide is mixed daily per the manufacturer's instructions. **Ensure all disinfectants being used are within their expiry limits.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • The following concerns were noted about the use of the steam sterilizer available on site for reprocessing semi-critical equipment.1. There were no chemical indicators in or on the pouches for chemical monitoring.2. Biological monitoring is being carried out once in 6months as opposed once every month.3. Chemical and physical parameters are not being verified and recorded.
    • 23. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Non-Critical)?
      • Load numbers are not assigned or recorded for each batch of equipment being reprocessed in the steam sterilizer.
