    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures available upon request.- Ensure written procedures are filled out and readily available. Operator will be sent written procedure template.
    • 20. Is reusable equipment disinfected properly?
      • Disinfecting procedure involved use of 70% isopropyl alcohol. - Discontinue the practice of using isopropyl alcohol for disinfecting and acquire a disinfectant with a DIN.- Operator went and acquired CS20, PREempt RTU, and a soaking container for CS20. Discussed with operator on how to use CS20 and ensure soaking of 20 minutes for semi critical tools. RTU can be used for non-critical tools.
    • 20. Is reusable equipment disinfected properly?
      • No testing or record keeping for high level disinfectant.- Acquire test strips for high level disinfectant and ensure record log is maintained for each test.- Operator did go out to get test strips but supplier did not carry them at the time. Please follow up with PHI once acquired.
