- Single use nail files observed used throughout the facility and being reused. Ensure all single use items are discarded after use. Failure to do so will result in enforcement action.

- High Level disinfectant must be tested and recorded. Please obtain test strips for CS20 and record testing. PHI will send log sheets.
- 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
- Workstations require a deep clean, please clean drawers, workstations and protect all clean tools from contamination.

High-level disinfectant, preempt HLD5, was being diluted with water and had expired since March 2024.-Please procure and use effective high-level disinfectant to ensure proper cleaning and disinfection of tools and equipment.Do not dilute the high-level disinfectant with water, please follow manufacturer's instructions for usage to ensure product efficacy.2. Alcohol was used to sanitize reusable nail drills.-Please use disinfectant such as preempt Hld5 to disinfect such tool.
