- - Biological indicators (BIs) were expired since August 2022.- Physical parameters were not accurately documented sterilization time and temperature of 45 mins and 250F were documented, however the correct parameters for the cycles used 270F for 10 minutes.Corrective Actions:- Acquire and process new BIs.- Correctly document the sterilization temperature and the amount of time the sterilizer maintains that temperature.Recommended Actions:- Use AHS sterilization log for documentation of cycle parameters, since it allow for documentation of external, internal, type 5 chemical indicators, as well as sterilization temperature and time, instead of cycle time.
