      • In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. WASTE WATER BACKING UP THROUGH GREASE TRAP UNDER 3-COMP SINK. INSTRUCTED MANAGER TO CALL PLUMBER AND REPAIR.
    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. REMOVE CAT FROM PREMISES. UPDATE PEST CONTROL LOG BOOK. INCLUDE COPY OF PEST CONTROL LICENSE, MAP OF TREATED AREAS, MSDS SHEETS FOR CHEMICALS USED.
    • 19. OUTSIDE GARBAGE WASTE GREASE AND STORAGE AREA; CLEAN, RODENT PROOF, ALL CONTAINERS COVERED
      • The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GARBAGE AREA NOT CLEAN. PROVIDE DUMPSTER FOR GARBAGE DISPOSAL.
    • 34. FLOORS: CONSTRUCTED PER CODE, CLEANED, GOOD REPAIR, COVING INSTALLED, DUST-LESS CLEANING METHODS USED
      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR MISSING AND DAMAGED FLOOR TILES IN ALL AREAS. FLOORS NEED CLEANING IN REAR BASEMENT (CAT FECES).

      • The walls and ceilings shall be in good repair and easily cleaned. SEAL OPENINGS AROUND PIPE FITTING UNDER SINK IN WASHROOM. INSTALL SPLASH GUARD BETWEEN 3-COMP AND EXPOSED SINK. WALL MUST BE SMOOTH AND EASILY CLEANABLE BEHIND 3-COMP SINK. SEAL ALL OPENINGS IN WALLS AND CEILINGS AROUND PIPE FITTINGS, ETC IN ALL AREAS. REPLACE DAMAGED CEILING TILES IN ALL AREAS. CLEAN WALLS IN ALL AREAS (SPILLS).
    • 36. LIGHTING: REQUIRED MINIMUM FOOT-CANDLES OF LIGHT PROVIDED, FIXTURES SHIELDED
      • Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. REPLACE DAMAGED LIGHT SHIELDS IN ALL AREAS.
    • 37. TOILET ROOM DOORS SELF CLOSING: DRESSING ROOMS WITH LOCKERS PROVIDED: COMPLETE SEPARATION FROM LIVING/SLEEPING QUARTERS
      • Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. INSTALL SELF-CLOSING DEVICE ON WASHROOM DOOR.
