    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1.Classroom hooks were installed too closely together (approximately 8 inches apart). Personal belongings, such as jackets, were observed touching between children.Increase the spacing between hooks to prevent contact between children’s personal items. Alternatively, implement measures to ensure belongings remain separated (e.g., designated bins or spacing strategies).

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Bleach sanitizer solutions tested at excessively high concentrations (500–1000 ppm chlorine) in all classrooms. The required concentration for general sanitizing is 100 ppm chlorine.Please prepare new chlorine solution. Guidance documents were provided.2. Items that are not age-appropriate were noted in the Explorer 2 classroom. These included bingo dabbers labeled for ages 12+ and small toys that can fit through a toilet paper roll, posing a choking hazard.The staff member removed the items during the inspection.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. During observation of diapering procedures in the Explorers 1 room, the staff member did not remove disposable gloves after removing a wet diaper and used the same gloves to dress the child and assist with handwashing, resulting in a risk of contamination. Diapering procedures were reviewed on site. Guidance document also provided.2. The diapering mat is not secured to the countertop, and the surface is slippery, creating a risk of potential injury or fall.Secure the diapering mat firmly to the countertop to prevent slipping or movement during diapering.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Staff were not knowledgeable about proper toy cleaning and sanitizing procedures. The staff member stated toys are soaked in bleach solution without first washing with soap and water or rinsing. The staff member was also unable to explain the correct bleach sanitizer concentration.Provide staff training on proper toy cleaning procedures, including correct sanitizer preparation and concentrations. Guidance documents were provided.
