- Observation(s): The "F'real" beverage dispensing unit has significant amount of accumulated stains and debris on surfaces in the food dispensing area. Take immediate corrective actions and contact the health officer by Feb. 15, 2019.

- Observation(s): The test kit for the chemical sanitizer is expired in 2015 as per the label. Take corrective actions and contact the health officer by Feb. 15, 2019.

- Observation(s): The chemical sanitizing solution prepared by the operator has over 400 parts per million (ppm) QUAT. Over five attempts by the operator the concentration was still over 400 ppm. The poster on the wall indicates that the required concentration is 200 ppm. The health officer reviewed the chemical sanitizing testing requirements with the the operator during this inspection. Take immediate corrective actions and contact the health officer by Feb. 15, 2019.
