OBSERVED APPROXIMATELY 50 LIVE FRUIT FLIES ON WALLS AND CEILING OF THE DISHROOM, FOOD PREP AREA, LOWER LEVEL HALLWAY, LOCKER ROOMS AND RESTROOMS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED.       • The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE WALLS AROUND THE 3 COMPARTMENT POTWASH SINK AND THE DISHROOM. OBSERVED PEELING PAINT ON CEILING TILES AND CEILING TRACK IN THE DISHROOM AREA. OBSERVED AN OPENING IN THE WALL UNDER THE TOILET IN THE WOMEN'S LOCKER ROOM AND LOOSE WALL TILES UNDER THE TOILET IN THE MEN'S LOCKER ROOM. MUST REPAIR BOTH AREAS.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • OBSERVED A DRAIN LINE WATER LEAK BELOW THE LEFT COMPARTMENT OF THE 3 COMPARTMENT POTWASH SINK. MUST REPAIR. VENTILATION FANS IN THE LOWER LEVEL LOCKER ROOMS AND RESTROOMS ARE NOT OPERATIONAL. MUST REPAIR.

      • All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. CLEAN ALL CIGARETTE BUTTS FROM THE REAR RAMP AND REAR EXIT AREA TO THE ALLEY AND PARKING LOT.
