    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Operator had not notified AHS about their business. Worker spoke to the PHI about the business and description of services offered.PSN was completed by PHI offsite.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for skin cleansing, and accidental skin cuts and punctures were not established.Corrective actions:- Develop written procedures for skin cleansing and accident skin cuts and puncture.- AHS written procedure template may be used to assist in development of procedures.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Sink in the facial room was used for hand washing, according to personal service workers. The sink was supplied with liquid soap but not a sanitary method for drying hands.Corrective Action:- Install a paper towel dispenser and ensure the sink is always supplied with paper towels.
    • 20. Is reusable equipment disinfected properly?
      • Barbicide low level disinfectant containers were half full, and not changed daily, as required by the manufacturer.A high-level disinfectant was not available to disinfect semi-critical equipment (blackhead extraction loop).Corrective Actions:- Ensure Barbicide disinfectant is used according to the manufacturer's instructions for use (preclean combs, 10-minute full immersion, rinse, dry, and prepare fresh solutions daily)- Acquire a registered high-level disinfectant for use on semi-critical equipment such as blackhead extraction loops.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Staff were double dipping wax sticks for certain waxing services which do not involve waxing of the genital area.Corrective Action:- New wooden wax stick must be used each time wax is taken from the wax pot and placed on a client, regardless of the type and location of the service.
