- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Client contact information was collected as required. Sterility assurance documentation of Juvederm dermal fillers were collected along with the client's chart.Going forward, ensure that sterility assurance documentation (lot #s, expiry dates) for all other critical equipment such as insulin syringes used for injections, are also documented.

- Botox Cosmetic was stored in a locked fridge that was maintained at 5C, as per the thermometer affixed to the door.There were no opened vials of Botox, however the operator stated that once reconstituted, vials are labeled with the date and time, and unused product is discarded 24 hours post-reconstitution.*** Botox Cosmetic was reconstituted with bacteriostatic saline, which is contrary to what the manufacturer instructions for use (MIFU) requires.Ensure the MIFU is followed by reconstituting Botox Cosmetic with preservative free saline.
