- Multiple opened packages of LD‑00303 needles were found in the facility without information confirming their sterility; therefore, sterility cannot be verified.- Stop using the LD‑00303 needles immediately. A syringe with attached needle was observed stored loose in the fridge, the device is no longer sterile and unsuitable for patient use.- Syringes and needles are single‑use items and must be discarded immediately after use. Open sterile packages only at the point of use.

- Multiple expired products were found in the facility, including hyaluronidase (expired Mar 02, 2025, Sept 05, 2025, Sept 18, 2025), needles (expired May 2019) ointment (expired Jan 2019), sterile water (June 01, 2025). - Discard all the expired products. Opened vials of Xylocaine, bacteriostatic sodium chloride injection USP, triamcinolone acetonide injectable suspension USP, lidocaine hydrochloride injection USP, and sodium bicarbonate were found stored at room temperature without date‑of‑opening labels.- Follow all manufacturer instructions for storage and stability, label each vial with the date it was first opened and discard according to the manufacturer’s recommended beyond‑use datingAn Exohealer exosome product was found in the facility. The ingredient list includes “human cord blood cell conditioned media.”- As this product contains human‑derived ingredients and is not approved by Health Canada, the facility must immediately stop providing any services using this product.
