- Observation(s): LO Vermeulen reviewed person in care records with facility manager. Immunization status could not be located. Please ensure this information is present for all persons in care in order to comply with section 78(1)(a) of the RCR.

- Observation(s): LO Vermeulen reviewed documentation regarding PRNs given with facility manager. It was observed that PRN results were consistently not documented. It is a requirement that a person in care's reaction to the PRN be indicated after being administered each time. This contravention was noted on the two previous inspections. Please indicate how this practice will be resolved and what systems/audits will be put in place to ensure ongoing compliance with this section of the legislation.

- Observation(s): MARS were reviewed randomly during inspection. It was observed that PRN results are consistently documented as "monitoring." Please ensure a PICs reaction to the PRN is indicated as required. The intention of this section was discussed with the facility manager during the inspection. RCR 78(2)

- Observation(s): Personal service plans (PSPs) were reviewed for PICs. The use of a restraint was indicated in one PSP but current details of the restraint procedures as well as a system of monitoring the use of the restraint could not be located. Please ensure details of plan are updated and monitored as per RCR. 84.

- Observation(s): Shower in basement bathroom requires slip resistant material to prevent risk of injury.

- Observation(s): A system of self-monitoring is required to be developed that includes the physical environment and the care and services provided to PICs as per RCR 61. Details such as fridge temperature, elevator maintenance, water temperature, MARS and Medication Safety Advisory Committee meetings were discussed at time of inspection.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): Outside activity area on lower level of facility requires maintenance. The current state of the furniture and yard may be unappealing to PICs and not encourage its use. Grass was observed to be long and furniture covered with dust.
