      • **Repeat violation**- There are some holes in the roof where previous light fixture was installed nearby cubby area and in the*Repair the hole to ensure all the surfaces including walls, and ceiling are smooth, easily cleanable and non-absorbent.Update from 10/08/2025There are open pipes from an old vacuum system in the ceiling in the pre-school room. - Close the pipes.

Test strips to measure hydrogen peroxide strength (in PPM) is needed for using this disinfectant. Concentration should be 5000 ppm.

      • The preschool room and cubby area was cluttered.Organize the cubby area and the preschool room, remove any item/equipment that is no longer in use.
