      • -Operator had discontinued the use of a high level disinfectant and was instead using a sterilizer unit for re-useable tools. Due to inadequate monitoring of Biological, chemical and physical parameters this unit cannot be used. Please ensure that a high level disinfectant is used for re-useable tools.

      • -Wax rollers were observed. These are not allowed as they lead to contamination of the bulk supply. Please remove.
