- Facility specific written procedures for client care, skin cleansing and decontamination of dual purpose sink were not completed.Procedures were completed onsite.

- Single use wooden spatula was noted in the wax container. Operator was advised to remove the spatula and must discard after single use.

- Contact times of the disinfectants were reportedly not followed being used according to the manufacturer's instructions for use. Instructions from the labels of the disinfectant containers were reviewed with the operator onsite.Ensure contact times are followed to achieve adequate disinfection of the tools.

The solution was not reportedly changed according to the manufacturer's instructions for use(MIFU). Change the solution daily or often according to MIFU.Ensure the combs and other tools are fully immersed in the disinfectant solution.

- The wax container lid was missing. There was single use wooden spatula noted in the wax container, indicating double dipping.Provide lid to the wax container to prevent contamination and prevent double dipping to prevent contamination of bulk supply.

- Hair wash stations were interchangeably being used for reprocessing non-critical tools without decontamination of the sink. Operator was advised to designate one hair wash station for reprocessing and complete follow written procedure of decontamination of dual purpose sink prior to reprocessing of the tools.
