    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written aftercare instructions for electrolysisEnsure there are written aftercare instructions provided to client, in hardcopy or electronically.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Needle information was not written on client's file. Ensure that sterility information is being recorded (manufacturer's name, LOT number, expiry date) for the electrolysis probes. Suggest attaching the packaging of the items onto the client consent form. Records should be kept for a minimum of 2 years.
    • 20. Is reusable equipment disinfected properly?
      • 1) Use of 70% alcohol for disinfecting tweezers, which is not allowed as it does not have a DIN. Please use immediate disinfectant such as PREempt RTU or high-level disinfectant such as PREempt CS20 for disinfection.2) Tape was used to secure a cracked or loose part of a probe holder. Tape can harbor bacteria and is not easily cleanable. Replace the probe holder or provide a cleanable cover.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Reprocessing of tools was done in the washroom. Please do not use the toilet and put sign on the toilet to ensure the basement toilet is not being used.
