      • 1. The string lights were not secured to the ceiling in the Bear room. A child grabbed a portion of the string lights while standing on top of the diapering table.Please secure the lights to the ceiling and ensure it is not accessible to children.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The storage cubbies located in the hallway are not mounted to the wall. The cubbies were 48 inches in height.Please mount the cubbies.2. The laundry washing machine had buildup of dirt and debris.Please wipe and sanitize the washing machine. Ensure it is in sanitary condition.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. The water tables in the Otter room have exposed metal drainage valves. Please consider adding tape or a cover over the drainage valve to prevent a potential finger injury.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • 1. Homemade playdoh was noted inside the Bison room.Homemade playdoh with raw flour is no longer allowed. Please remove the playdoh from the room.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Some bleach spray bottles were not labeled with its content. - Ensure that chemicals are adequately labeled. 2. Staff in the upstairs classroom was mixing bleach solution without verifying the concentration. The bleach solution tested very strong. The staff did not have test trips available.- When concentration of bleach solution is over 100 ppm, ensure that it is rinsed off any food contact surfaces and mouthed toys.

      • 1. Upstairs classroom: A classroom sink is being used for toy washing but the sink drain stopper did not work. - Repair so toys could be adequately washed and submerged in sanitizer. 2. 4yrs old room: One ceiling light did not have a protective cover. - Install 3. Some window screens were damaged in 4 and 5 yrs old rooms. - Repair or replace the screens. Ensure that windows are in good condition. 4. A bottom edging strip on a wooden toy shelf in 5 yrs old rom was damaged. - Repair so the surface is easy to clean.

    • 08. Is this facility properly constructed (Non-Critical)?
      • Bear classroom: The push bar for the door was in disrepair, tape was used to secure it in place.Please fix the push bar and remove all tape from the door. Wolves classroom: The metal transition strip stopped halfway in the classroom exposing a protruding sharp edge that can lead to injury. Please fix the transition strip and ensure the strip runs through the entire length of the floor.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The sensory table in the wolves classroom was unsanitary, dirt and dust were observed on the sensory table. Please clean the sensory table.
