- Following conditions were noted and corrected during the inspection:1) Few of the rooms had missing electric outlet covers.2) Small toys (possible choking hazard) were observed in the toddler room.3) Small equipment with long cords were observed in unlocked cabinets.4) Unlocked cabinets with arts and craft supplies in toddler roomOperator was very cooperative and took prompt action to address the noted concerns.

- 1. The 1000 ppm chlorine were in bottles labeled as 100 ppm. Some of the bottles were relabeled during inspection.- Please ensure all bottles are labeled correctly. Recommendation to have a system in place to make it easier to switch between 100 ppm to 1000 ppm labelling.

- 1. Infant room: The chlorine solution was tested and it only measured 500 ppm. A new solution was mixed during the inspection. - Please ensure all chlorine bottles measure at 1000 ppm during an outbreak.2. A high-touch cleaning checklist was not available upon request. It is difficult to verify how often cleaning and disinfection is being conducted for high touch areas like door knobs, light switches etc. Staff were also giving vague answers when questioned.- Please have a high touch cleaning record on site and ensure it is completed.

- 1. Sr. Movement room: Books were left out in the room. The operator stated hard covered books are currently being left out so it can be wiped down.The inside pages are unable to be disinfected.- Please remove the books from the room until the outbreak is declared over.

- 1. Infant room: The diapering area/surface had a peel and stick adhesive. The adhesive was cracking and peeling off making it difficult to clean. There was an accumulation of dirt and debris under the adhesive. Staff removed the adhesive during the inspection and sanitized the surface.Please make sure the diapering surface is easy to clean.

- 1. Senior movement room: One of the blankets stored between the cots was hanging down and it was touching the floor. The corner of the blanket had a build-up of debris. Staff removed the blanket during the inspection.Please ensure bedding is stored in a sanitary manner at all times.

Please remove rugs to allow more easily cleanable floor surfaces during outbreak.3. Mop heads are laundered on a weekly basis. Please clean and disinfect mop heads on a daily basis even when outbreak is declared over.

- 1. The spaghetti lights now hanging on the wall in the sensory room still pose entanglement hazard. Please either remove them or bundle them together securely. Removing them is strongly advised.

- 1. Staff did not follow hand hygiene before dressing the child after discarding dirty gloves and diapers. Please replace the expired hand sanitizers and consider using hand sanitizer between steps of donning and doffing gloves when hands are not soiled.

- 1. The spaghetti lights in the sensory room pose entanglement hazard as the tape to bundle them together has came off. Please remove lights or secure them.2. Several bins for toys storage were not in clean condition. Please clean and disinfect the bins before placing clean and disinfected toys back into the bins. 3. Wicker basket is made of cleanable material. Please provide liner that can be laundered or replace with one that is cleanable.

- 1. Staff did not conduct hand hygiene before changing diaper for the second child. Please ensure hand hygiene is followed after each diaper change. Placing a hand sanitizer by each diaper change table is strongly advised. 2. Staff lathered her hands with soap under running water. Please ensure hands are thoroughly lathered before rinsing under running water.3. Gloves were used for cleaning tasks but not changed between tasks. Please ensure staff remember to change gloves between tasks when gloves are used (i.e. cleaning vs. setting place mats).
