- 1. Ice scoop stored inside the ice making machine had the handle directly touching the ice. Do not store the scoop in the ice machine, store in a clean container2. Crate on floor containing mesh bag of onions. Containers of bulk spices were also stored on floor level. Ensure all food containers are stored off the floor at least 6 inches.

- 2 bottles of prepared chlorine sanitizer were unlabeled to identify its use or contents. Ensure all chemical bottles are labeled for proper use.

- 1. Thermometer missing in the coolers. Provide a thermometer inside the coolers for monitoring food storage temperature is at or below 4 deg C.2. No probe thermometer onsite for checking internal food temperatures for cold & hot holding, cooking or cooling procedures. Obtain a probe thermometer for use onsite.

- Wall and floors need cleaning around cookline area.Wall mounted shelf at kitchen entrance is grease laden. Please clean these areas on regular frequency to prevent buildup of grease.
