Sterility assurance documentation (such as tattoo and piercing needles lot number, expiry date and manufacturer's name) was not being kept for all pre-purchased critical equipment.-Please maintain the indicated information for all pre-purchased sterile equipment with client contact information and accessible for at least two years.

      • Load numbers were not assigned/labeled and recorded for/on every sterilized package.-Please label, assign and record the lot numbers for very sterilized package so that every package can be traced back to sterility qualification records.

      • Some expired tattoo ink bottles and needles were seen onsite.-Do not use expired products, always follow manufacturer's instructions. Please remove and discarded all expired products.
