      • Written procedures for skin cleansing, antiseptic application, failed sterility and in an event of accidental cut or puncture were not available onsite.-Please provide onsite the indicated written procedures.     • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • 1. Load numbers and verified chemical indicator results for all in-house sterilized critical equipment were not maintained/kept with client information.-Please maintain the indicated records for all clients, ensuring the information is accessible for at least two years.2. Sterility assurance documentation (such as tattoo and piercing needles lot number, expiry date and manufacturer's name) was not being kept for all pre-purchased critical equipment.-Please maintain the indicated information for all pre-purchased sterile equipment with client contact information and accessible for at least two years.
    • 23. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Non-Critical)?
      • Load numbers were not assigned/labeled and recorded for/on every sterilized package.-Please label, assign and record the lot numbers for very sterilized package so that every package can be traced back to sterility qualification records.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some expired tattoo ink bottles and needles were seen onsite.-Do not use expired products, always follow manufacturer's instructions. Please remove and discarded all expired products.
