- 1. Pre Empt RTU to disinfect the non-critical surfaces was expired (Oct 2025). Please purchase new supplied asap.2. Pre Empt CS20 bottle was not available (running out of supplies). Facility had some of the CS20 pour inside the container at the reprocessing area. Please purchase the supply ASAP. 3. High level disinfection logs were not maintained properly, sheets were incomplete and logs were not maintained after June 2025. Instructions provided regarding completing the high-level disinfection logs.4. Test papers for high level disinfectant (AHP 7%) were expired. Please purchase new supplies.

- Spoons are being stored in foot scrubs, and the spoon handle was in contact with the scrub. To prevent cross-contamination, use disposable or single-use scoops. Do not store spoons in scrubs.

- The container for disinfecting tools was not big enough for foot paddles. Foot paddles are being just sprayed with disinfectant. - Food paddles must be soaked in a high level disinfectant after each use.- Store soaking container beside reprocessing sink to have easy access to the disinfectant.
