    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Chipped wooden playing blocks were observed in the preschool 2 room.Please sift through the wooden blocks and sand or remove any chipped blocks to prevent splinters.
    • 06. Are proper protocols being followed in regards to client care?
      • Bibs were observed hanging above the handwash sink used for diaper changing.The operator was instructed to remove them from the area and to clean and sanitize the bibs. Please find another location to hang or store the bibs.

      • 1) The diaper straps located in the infant room were soiled with stains and debris.Please clean and sanitize the straps.2) The diaper changing mat located in the toddler 2 room was starting to wear, exposing the foam parts making it difficult to clean and sanitize.Please replace the diaper changing mat.

      • A cushion located in the toddler 1 room was observed to be dirty and soiled with ink.The operator promptly removed the pillow.

      • 1) The caulking behind the handwash sink located in the toddler 1 room was in disrepair.Please remove the old caulking and seal the joint with a new bead.2) The grooves and crevices around the handwash sink located in the infant room was dirty and covered in brown stains.Please deep clean the crevices and sanitize the surfaces.3) The caulking being the handwash sink located in the toddler 2 room was dirty and stained.Please deep clean the caulking and sanitize the surfaces.4) The grey rug with long fibres located in the toddler 2 room was matted and trapping dirty and contaminants.Please replace the rug with one that is easily cleanable.

      • Evidence of mouse droppings were observed beside the handwash sink located in the preschool 1 room.Please carefully clean and disinfect the droppings and locate any holes and gaps and fill them with steel wool, then seal them to prevent the entry of unwanted pests. Apply appropriate treatment to address the pest issue, then monitor and adjust as needed. Provide an updated pest control report to the undersigned PHI.

      • 1) The leather on the brown couch in Room 5 is damaged in multiple areas and cannot be properly sanitized.*Please replace or repair the cover to ensure proper sanitation.2) The wooden table in Room 5 is missing two screws and is unstable, posing a safety hazard.*Please remove the table from the area or repair it.

      • - Diapering stations in infant/baby room was not adjacent to dedicated handwash sinks. *Reposition the diapering stations to be within access to handwash sink for use.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Safety locks for cupboards below sinks and diapering stations (where ointments were store) were not locked properly. Corrected onsite. Ensure staff ensure safety locks are locked properly after use. 2. Room 5- the following items stored on the wall ledge had cords accessible for children (Globe, phone base). Corrected onsite. Ensure items stored on the ledge are not accessible to prevent potential tip-over and injury.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Diapering stations in Room 2 & 3 are not adjacent to dedicated handwash sinks. Reposition the diapering stations to be within access to handwash sink for use. 2. Diaper pads in Room 3 & 2 -safety belt missing clip on one end. Pad in Rm 3 also has torn surface cover. Replace both diaper pads. 3. Diaper station in Room 2- some dust collecting on underside corners of diaper pad. Ensure after diapering, the diaper pad and surrounding surfaces of the diaper station are cleaned and disinfected properly.

      • First aid kits in back packs for outdoor use are missing items (CPR barrier device, antiseptic wipes). Refer to OHS CSA Standard Z1220-17Type 2 Basic Small First Aid Kit for medium hazard work setting for full list of items.

      • Infant room- wall missing corner guard. Edging of wall panel exposed on the corner. Please install a corner guard
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Hot water tank capacity is inadequate for demand supply needed after peak use from both kitchen and washrooms. Recharge of tank is 45 minutes and periods of no hot water occur before recharge completed. Measured hot water at tap measuring at 44 deg C but depleted after heavy use during inspection.Facility is currently working on adding additional hot water supply to supplement use onsite. Discussed onsite installation for instant demand unit for kitchen use to separate supply from washroom sinks.

      • Room 2- exterior door in room is missing weather strip along bottom edge of door. Visible light observed. Ensure a weather strip is installed to prevent a gap along the bottom edge of door.
