    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Personal Service Notification was submitted during the inspection, as directed by the PHI.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • The written procedure templates for1) Client skin preparation2)Accidental Cuts and PuncturesWere provided during the inspection.This violation was corrected during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • During the inspection it was observed that operator was using Pre-empt Concentrate (Intermediate Level Disinfectant) for disinfecting semi-critical tools (Equipment that comes into contact with mucous membrane or non-intact skin but is not intended to puncture either of them)High Level Disinfectant was not available.Operator was verbally ordered to stop providing any service involving semi-critical equipment to client until HLD is available.Note: Operator mentioned that she would be discarding the HLD after each use.
    • 24. Are waste sharps disposed of properly?
      • Sharps container was not available for safely disposing the single use sharp blades.Sharps are considered the bio-hazard and must be safely discarded in appropriate sharps container, to reduce the risk of infection.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • It was observed that one of the product used for eye-brow tinting was labelled as "not suitable for use near or around eye area"Operator was instructed to stop using these products.Make sure that cosmetic products are appropriate for their intended use and follow manufacturer's instructions for cosmetic products.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • Operator was using either the pedicure basin or the hair washing sink for tools cleaning and reprocessing.Procedures were not avaliable for sink decontamination between use for dual purpose.Another sink was available in the laundry room.Discussed with operator to dedicate the sink in laundry room for reprocessing of semi-critical tools.Operator confirmed that laundry room sink would be modified for tools cleaning and reprocessing.Please make sure that there is sufficient work surface area on either side of the sink to facilitate a one way workflow to separate contaminated equipment from clean equipment and to facilitate air drying of reprocessed equipment.
