      • Written procedures for skin cleansing, accidental skin cuts/ punctures and decontamination of dual-purpose sink were not available.Complete written procedures. AHS templates were provided.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Hair and floating debris were observed in the CS20 solution stored in the disinfectant tray.Contaminated disinfectant was discarded during the inspection. Clean equipment properly before disinfection.
    • 20. Is reusable equipment disinfected properly?
      • 1) Reusable equipment was soaked in the CS20 solution for approximately 15 minutes. Submerge equipment for minimum 20 minutes as per manufacturer's instructions.2) Test strips were not available to monitor the effectiveness of CS20 during the reuse period.Provide test strips and monitor the disinfectant daily during the reuse period.3) A high-level disinfectant log was not maintained. Maintain daily records.4) The dual-purpose sink was not disinfected between handwashing and equipment reprocessing.Clean and disinfect the sink between different uses.
