- Sterility assurance documentation such as lot number, expiry date and manufacturer's name for pre-purchased sterile equipment (such as tattoo needles and piercing needles) was not available or kept for all clients that undergo invasive procedure. -Please kept the indicated documentation with client contact information and accessible for at least two years.

- Class 5 Chemical indicator was not being packaged as required. The class 5 chemical indicator was to be packaged if the sterilized item was packaged.-Please ensure the chemical indicator included in the sterilized load is packaged if the items being sterilized are packaged.

- Load number was not assigned to every sterilization tray load and every sterilized packaged.-Please assign load number to every sterilized load and package, these sterilization load number must also be properly documented.

- Water used to mix tattoo ink was not sterile at the point of use. Filtered municipal supplied water was used to mix tattoo ink.-Please provide and use sterile water as indicated to meet Personal Services Standards.
