      • Sanitizer test strip not available to monitor and ensure concentration adequacy.-Please provide suitable test strip and ensure daily monitor of sanitizer concentration.

      • There was no guardrail or gate available at the top of a stairway in close proximity to children's classrooms.-Please provide securely installed and effective guardrail or gate to the indicated area.

      • 1. Small hole noted beside electrical socket in 12-19 months classroom.-Please fix and ensure good repair.2. All facility's classroom floors are covered with rugs which does not meet the regulatory requirements.-Please provide a segment in each classroom with smooth, impervious to moisture and easy-to-clean floor surface as children eating/food service area.
