    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • The operator is using the sink in the service room to perform hand hygiene and to reprocess semi-critical tools. There are no written procedures for decontamination of a dual-purpose sink. Written procedures are required for decontamination of a dual-purpose sink. An AHS template was provided to the operator.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • After the semi-critical tools (comodone extractor) is disinfected, the operator is placing it in a small jar and the equipment is exposed to potential contamination (dust/debris). Reusable tools must be stored in a manner that protects it from contamination. Cover the tools to protect it from contamination or place the tools inside a clean container.
    • 20. Is reusable equipment disinfected properly?
      • 1. The operator is using alcohol to disinfect non-critical tools (the head piece of the high frequency equipment). Alcohol is not considered a disinfectant. Use a low-level or intermediate level disinfectant for non-critical devices. 2. The operator disinfects the semi-critical tool (comodone extractor) with the high-level disinfectant, then places it in alcohol. For semi-critical tools, use a high-level disinfectant and ensure the manufacturer's instructions are followed accordingly.3. There is tape used to cover the hand piece of the high-frequency equipment. The operator stated the equipment is not broken. The tape is used to prevent water from going down the hand piece. Tape should not be used as it does not allow proper cleaning and disinfection for non-critical tools and equipment. Remove the tape.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • The hand sink was not equipped with paper towels to dry the hands. Paper towels must be placed at the hand sink to perform proper hand hygiene. Please install.
