      • The water temperatures of the handwash sinks located in the infant room and the toddler 1 room (diaper changing sink) were tested using a probe thermometer and measured at 57C and 58C respectively.Please adjust the water temperature to a maximum of 54C, recommended 49C.

      • The exit sign located in the toddler 1 room was observed to be crooked and loosely secured to its bracket, posing a potential safety hazard.Please repair the exit sign and ensure it is secured to the bracket to prevent accidental injury.

      • Water stains were observed on ceiling tiles located in the infant room sleeping area (corner closest to the entrance, and in the middle of the room).Please address the leak and replace the tiles.

      • When asked to demonstrate how the facility disinfects the surface, the operator would use water to rinse the surfaces after the disinfecting step, deeming disinfection ineffective.The operator was instructed to use a 100 - 200 ppm chlorine solution to disinfect surfaces and allow them to air dry, ensuring a 60 second contact time. If required, the two-wipe method may be used by cleaning heavily soiled tables with detergent and water before disinfecting.

      • The chlorine solution throughout the facility was measured at 1000 ppm and was being used on all surfaces despite the facility not experiencing an outbreak.The operator was instructed on how to properly mix a chlorine solution with a concentration of 100-200ppm. All chlorine solution spray bottles throughout the facility were remade to a concentration of 100-200ppm.During an outbreak it is required to increase the concentration to 1000ppm for enhanced disinfecting. The kitchen sanitizer solution must remain at 100-200ppm.

      • 1. Small Legos and crayons were seen in preschool room. This can be a choking hazard. Please remove.2. Shelf by the window in the toddler 1 room is not secured. Please secure.

      • Cover is missing from the garbage can's foot pedal in the preschool room. A sharp metal is exposed as a result of it. Please repair/replace.

      • 1. Mouthed toy bin in the preschool classroom was used for multiple purposes. Please ensure mouthed toy bin is used to store dirty toys only.2. Some foam toys in the preschool room have deep bite marks and not easily cleanable. Please discard.
