      • 1. Bleach solution was noted to be at 250 ppm. Please increase the concentration to 1000 ppm. Please use a test strip to test the concentration. Please increase frequency of disinfection of al high touch surfaces. 2. Visitors were not being informed of GI outbreak. Please be advised to inform all parents, visitors and staff members that the facility is under GI outbreak.
    • 06. Are proper protocols being followed in regards to client care?
      • One of the staff members was noted to be changing diaper and safety belt was not being used. The staff member was in the middle of the cleaning the client and director advised the staff member to put on the safety belt. The staff member proceeded to grab the safety belt made of velcro without performing hand hygiene. PHI advised the staff members to remove gloves and performed hand hygiene. Please train staff members with proper diaper changing procedures and hand hygiene.

      • A client's hands was observed to not be washed and dried after diapering in toddler room 2. The client was stopped immediately and their hands were thoroughly washed. Ensure the client's hands are thoroughly washed and dried after any diapering procedure.

      • Bookshelves are being used to subdivide the large room into multiple areas. The backside of these bookshelves is constructed of press board, which appears to be water resistant, however it is quite visibly dirty and requires deep cleaning. Any damaged sections which are no longer smooth or easy to clean must be replaced.

      • High level dusting is required on ceiling ventilation grates and surrounding ceiling tiles throughout the facility.

      • Increased monitoring is required in the infant room to adequately track which toys have been mouthed and thus should be removed and sanitized prior to being played with again.

      • There was a package of chemical surface sanitizing wipes in an unlocked cupboard under the handwashing sink in toddler room 3. This cupboard was at toddler level and posed a risk as the children could easily access the sanitizing wipes.

      • Trash bins in the facility had lids on them which prevented disposal of waste without manually lifting the lid beforehand. This was observed in the diapering area of toddler room 1, the diapering area of toddler room 3, and the OSC boys bathroom.

      • The paint in preschool room 1 was worn off and flaking along one of the walls. The flaking paint could be reached by children and further picked away. Paint chips were also observed on the carpet below this wall which pose a risk for children as they may put these pieces in their mouths

      • In toddler room 2 there was a computer keyboard being used as a toy. Several keyboard keys were noted to already be missing. This keyboard posed a choking hazard for the children if they detached more keys from it while playing with it as they may mouth them. The operator removed the keyboard during the inspection

      • There is visible buildup of scum and mold on the bathroom sinks throughout the facility. Sinks needs to be scrubbed thoroughly to remove the buildup in addition to the regular santizing.
