- 1. Bleach solution was noted to be at 250 ppm. Please increase the concentration to 1000 ppm. Please use a test strip to test the concentration. Please increase frequency of disinfection of al high touch surfaces. 2. Visitors were not being informed of GI outbreak. Please be advised to inform all parents, visitors and staff members that the facility is under GI outbreak.

- One of the staff members was noted to be changing diaper and safety belt was not being used. The staff member was in the middle of the cleaning the client and director advised the staff member to put on the safety belt. The staff member proceeded to grab the safety belt made of velcro without performing hand hygiene. PHI advised the staff members to remove gloves and performed hand hygiene. Please train staff members with proper diaper changing procedures and hand hygiene.

- There was a package of chemical surface sanitizing wipes in an unlocked cupboard under the handwashing sink in toddler room 3. This cupboard was at toddler level and posed a risk as the children could easily access the sanitizing wipes.

- Trash bins in the facility had lids on them which prevented disposal of waste without manually lifting the lid beforehand. This was observed in the diapering area of toddler room 1, the diapering area of toddler room 3, and the OSC boys bathroom.

- The paint in preschool room 1 was worn off and flaking along one of the walls. The flaking paint could be reached by children and further picked away. Paint chips were also observed on the carpet below this wall which pose a risk for children as they may put these pieces in their mouths

This keyboard posed a choking hazard for the children if they detached more keys from it while playing with it as they may mouth them. The operator removed the keyboard during the inspection
