- 1. It was noted that the staff member was not following proper diapering procedures. Proper diapering procedures were discussed with the operator during the inspection. Please train staff members. 2. Water bottles, food bowls and cereal were noted on the diapering counter. All these were removed during the inspection. Please do not anything on diapering station.

- It was noted that the facility was performing diapering on the floor of the Kinder washroom. A diapering station from the Toddler room (currently not being used but would start using this room on Sept 2, 2025) was moved in the Kinder washroom during the inspection. The operator also moved diaper genie from the baby room 1 in this washroom. Please acquire a diapering station for Toddler room and diaper genie for baby room 1.

- Bedding was noted on cots at the time of inspection. The bedding was moved to clients' cubbies during the inspection. The operator was advised to disinfect cots after each use. The operator indicated that they would store bedding in client's cubbies moving forward and all cots would be disinfected.

Please ensure that a disinfectant is present at all diapering stations. 3. Test strips to test concentration of bleach solution were not observed. The facility had test strips but were not bleach. Please acquire appropriate test strips.

- 1. The plumbing line of the sink located in the infant room was leaking. Please repair the leaking line. 2. An electrical switch plate was missing under the ink in the same infant room. Please install the missing electrical switch plate with GFI. 3. The wall area was not smooth in the same infant room underneath the sink. Please finfish the wall area.4. Cubbies were not properly sealed. Please seal all cubbies.

- 1. Diapering pad were not observed in Toddler and one of the baby room. Please acquire diapering pads. 2. Both baby rooms did not have barriers at diapering station for fall protection. Please install fall protections. 3. Test to test concentration of bleach solution were not observed. The facility had test strips but were not bleach. Please acquire appropriate test strips.

- 1. Holes were noted in almost all cubbies. Please seal these holes and ensure they are maintained in smooth, easy to clean and moisture resistant state. 2. A baseboard was noted to be missing from baby room 1 behind the cubbies. Bare dry wall was noted. Please seal the bare drywall.3. Porous ceiling tiles were noted throughout the facility. Some of these tiles were noted to be damaged. Please ensure that the ceiling must be made of material that is impervious to moisture. Please replace ceiling tiles.4. According to floor plan submitted and approved, a designated toy washing sink is located in the kitchen area. This sink was not observed during the inspection. Please install a toy washing sink as per the floor plan.5. The shelving units were not mounted to either floor or walls in all rooms. The operator stated that they are planning on moving these shelving units to accommodate sleeping.Please note that all shelving units must be either mounted to the walls or floor. These units must not be moved to accommodate sleeping. Please either floor mount or wall mount these shelving units in all rooms.6. The facility physical setup does not match the floor plan submitted and approved. According to the floor plan, a hand washing sink is located outside the washroom in the pre-school room and one outside of the staff washroom. These sinks were not observed during the inspection. Please install these sinks.8. The floor plans were not submitted to EPH prior to construction. A copy of the floor plan must be submitted prior to construction for review.
